Identify the need
Meet with stakeholders including ERMHS team, district site administrators, and teachers who currently have students receiving ERMHS. Determine current barriers to the referral process, implementation of service and access to resources. Goal of increasing streamlined support to services for students. -
Collect Data
Using google forms to develop a survey to send out to all district staff in order to determine current access to ERMHS, referral process and inform development of the plan moving forward. -
Analyze Data
Work with ERMHS team and other key stakeholders to determine reported needs to improve access to ERMHS. -
Create the vision
ERMHS team to use data to determine what information to include on internal website using google forms. -
Development
ERMHS team and IT support begin development of internal website to include referral procedures, documents and mental health resources in order to ensure student access to services. -
Progress Monitoring
Meet with ERMHS team and key stakeholders to monitor progress and test functions of the site forms. -
Implementation
ERMHS team meet with site administrators and leadership teams to introduce the internal site. Demonstrate the site and use of forms. Solicit feedback. -
Revise
Meet with ERMHS team and key stakeholders to revise based on implementation feedback as needed. Prepare for rollout in 2019/2020 school year. -
Rollout district wide
Send out link to all employees with welcome letter to include information regarding ERMHS, use of forms, referrals and resources. Focus on improved access to support for students. -
