  • First Mention of Amending Birth Records

    The given name may be added or amended and slight corrections made when supplemental reports or documental evidence is filed in said state bureau.
  • Amendments Expanded

    The State Board may make additions to or corrections in a certificate of birth upon receipt of adequate documentary evidence.
  • DNA

    The State Board may make additions to or corrections in a certificate of birth upon receipt of adequate documentary evidence, including the results of a DNA test
  • DNA Limitations

    The ISDH may make an addition to a birth certificate based on the results of a DNA test only if:
    1. A father is not named on the birth certificate; and
    2. A citation to this subsection as the authority for the addition is noted on the certificate.
