SPED Task 2- Timeline

  • First Talk About Transition Plan

    Talk to student about transition plan process (identify goals-plan & put in IEP-Graduate-post-secondary). Explain that the first step is to find out what the student is interested in after graduating high school.
  • Letter to Inform Parents of Transition Plan Process

    Send home a letter to the parents to explain the transition plan process (identify goals-plan & put in IEP-Graduate-post-secondary). Explain that the first step is to find out what the student and parents are interested in after graduating high school.
  • Visit Career Center

    Visit the carrer center to talk to the career counselr and research career options
  • Meeting to Find Student's Career Interests

    The student will complete the Career Cluster Interest Survey, and the Career Interest Survey.
  • Meeting With Sudent for Transition Plan Survey

    The student will complete a survey about measurable post-secondary goals, present levels of performance,coordinated set of activities, and the development of transition goals. The student will also complete a Strengths Based Assessment.
  • Meeting With Parents to Complete Surveys

    Meet with parents to complete Transition Plan Survey and Strength Based Assessment.
  • Notify Parents of IEP Meeting

    Notify Parents of IEP meeting on October 22nd.
  • IEP Meeting to Discuss and Establish Transition Plan

    The IEP team, the student, the parents, and a representative from The Minnesota Department of Rehabilitation Services will meet to discuss and establish the transition plan goals and objectives.
  • Write new IEP with Transition Plan

    The new IEP will be written to include the transition plan.
  • Notice of IEP Change to Parents

    A letter will be sent to the parents to notify and get permission for the new IEP.
  • Student turns 14. New IEP in Place

    The student turns 14, and the new IEP is put in place with the transition plan.
  • Behaviorl/Social Supports Put In Place

    The student will begin recieving services from SPED staff for behavioral and social skill support due to the EBD diagnosis, and in accordance with the transition plan.
  • Student Begins SPED Services for College/Career

    The students will begin recieving instruction from the SPED staff on comprehension of the Dunwoody College of Technology enrollment process, and training on using the MTC Bus System.
  • Notify Parents of Annual IEP Re-Evaluation

    Notify Parents of Annual IEP Re-Evaluation
  • Annual IEP Review

    Annual IEP Review Including-
    Transition Assessments

    The student enters his senior year of High School.
  • Visit Mechanic

    The student will spend one-two hours with the mechanic, **** ***** *********** ********* ********* **** ****, to learn on-the-job.
  • Inform Parents of Annual IEP Review

    Inform Parents of Annual IEP Review on November 5th.

  • Visit Dunwoody College of Technology

    The student will visit Dunwoody College of Technology in Minneapolis, MN to tour the campus and ask questions.
  • Deadline for IEP Objectives

    The student will demonstrate mastery of the skills for understanding the Dunwoody student handbook, and the MTC bus schedule outlined in question #4 of this assignment.
  • Graduation

    The student graduates high school.
  • Move Into Apartment

    The student will move into his aprtment.
  • Begin On-Campus Job

    The student will begin his on-campus job.
  • Begin Classes at Dunwoody

    The student will begin classes at Dunwoody College of Technology .
  • Meeting to Inform Student Of Transfer of Parental Rights

    The student turns 18 and will be informed of the transfer of parental rights.
