Act IV Scene III - Juliet and Friar Laurence form a plan where she takes a potion the stimulates death so she can sneak away with Romeo. Friar John is tasked to give a letter to Romeo that says that she has taken a potion and will sneak away with him, where he later doesn't deliver it to him. -

Act V Scene I - After finding out about Juliet's death, Romeo decides that he must die with the one he loves -
The Death
Act V Scene III - Romeo arrives at Juliet's tomb to die with her. Paris confronts him. Romeo kills Paris. He lies with Juliet and drinks the poison, killing him. Juliet's potion wears off and she wakes up and sees his body. She then takes Romeo's dagger and stabs herself in order to die with him. Both houses see their children's death and vow to end their hatred
