After his daughter and law, Elena was dressed inappropriately according to Ivan, he attacked her causing a miscarriage. This then lead to Ivan Ivanovich (Ivan the terrible's oldest son) to defend his wife and get killed. His emotional distress most likely lead to murdering his son. -
