An OC Transpo double-decker bus collided with a Via Rail train, killing six people and injuring dozens. The cause of the collision is still under investigation. -

Reported OC Transpo bus incidents at the Fallowfield station

Incident 1
A northbound OC Transpo bus drove through the railway crossing when the lights were activated at about 7:45 a.m. The lights reportedly went on when the driver was 30 feet away. -
Incident 2
A southbound OC Transpo bus drove through the lights at about 6:48 p.m., but this time it was about 15 feet away and it was driving at 60 km/h – or 10 km/h over the speed limit. -

A southbound bus drove through the activated lights at about 7:40 p.m. -

An OC Transpo bus drove through the activated rail lights southbound at about 7:45 a.m. -
Incident 5
At 10:16 p.m., an OC Transpo bus driver approached the crossing on the southwest Transityway to find that just one crossing arm was in the down position and the signal lights were activated. There was no train approaching. Buses were rerouted and RailTerm was called in to fix the problem. -
Incident 6
At 6:54 a.m., the City of Ottawa sent out a memo to say that the Via Rail railway signals at Fallowfield between Greenbank and Woodroffe are malfunctioning. Police were called in for traffic control and RailTerm was called in to fix the issue. -
Incident 7
Ottawa police are called back to the site just after 9 p.m. when the signal lights and arms malfunctioned again. Maintenance staff finished up the repairs by 10:45 p.m. -
"Routine maintenance"
The city closed the southwest Transitway overnight between Fallowfield Road and Woodroffe Avenue for "routine maintenance work." -
Incident 8
Warning signals flashed and the gates came down at the Fallowfield Road railway crossing, but there were no trains approaching.
