Issuance

  • State Begins to Issue

    The state shall, upon request, furnish any applicant a certified copy of the record of any birth or death registered under the provisions of this act, and such copy of the record, when properly certified by the secretary of said board to be a true copy thereof, shall be prima facie evidence in all courts and places of the facts therein stated.
  • Free Certificates

    No charge shall be made by any health officer for furnishing either a birth or death certificate when needed in connection with:
    1. His or her service in the US Armed Forces
    2. Death Pension (US Government)
    3. Disability Pension (US Government)
    4. Work Permits
  • Fee Established

    Any Person desiring to obtain a certified copy of a certificate of either a death or a birth may obtain same from city or county health officer. Charge not to exceed $1.
  • State Issuance Requires Direct Interest

    The state registrar shall not permit inspection of the records or issue a certified copy of a certificate or part thereof unless he is satisfied that the applicant has a direct interest in the matter recorded and that the information is necessary for the determination of personal or property rights.
  • Certificates Free for Members Of Armed Forces

    No charge shall be made by any health officer for furnishing either a birth or death certificate to a person in the US Armed Forces
  • Delayed Births Established

    The SBH shall issue a certificate of delayed birth registration without charge.
  • Local Records Still Issued to Anyone

    A LHO shall provide a certificate of birth, death, or stillbirth registration upon request by any person.
  • State Still Requires a Direct Interest

    The records and files of the SBH may only permit inspection or issue a certified copy if the applicant has a direct interest in the matter and it is necessary for the determination of person and property rights.
  • State Fees

    ISBH fees added:
    1. $3.00 ($18 today) search fee, if the requested record is found, 1 certification of the record will be issued without charge.

    2. Additional copies of the same record will be issued $1.00 ($6 today)
    3. $1.00 ($6 today) Correction fee

    The LHO and ISBH shall maintain a permanent record, accompanying the birth records, which contains for each request for certified copy of a birth certificate:
    1. The date of the request
    2. The name and address of the person making the request
    3. The signature of the person making the request

    The LHO may only issue a certificate if the officer is satisfied that the applicant has a direct interest in the matter requested and after the LHO determines that the record is necessary for the determination of personal or property rights.

  • Birth Problems Registry Fee

    The ISBH shall charge a birth problems registry fee of $2 for each search of the records for a birth certificate.

    The LHO shall maintain a permanent record, accompanying the birth records, which contains for each request for certified copy of a birth certificate:
    1. The date of the request
    2. The name and address and telephone number of the person making the request
    3. The signature of the person making the request
  • LHDs May Set Fees

    The local Health Department may now establish fees for certificates with local ordinances.
  • Coroner's Continuing Education Fee:

    If the LHD makes a charge for a certificate of death a $1 coroners continuing education fee must be added
  • Exclusion of Cause of Death

    Upon determination that a person may be provided a certification of death under subsection the local health officer shall provide to the person a certification of death that excludes information concerning the cause of death if the person requests the exclusion of this information.
