  • 4th Day

    On the 4th day of each month every city and county HO shall send the original birth, stillbirth and death certificates filed during the preceding month to the SBH in the furnished envelopes.

  • On the 4th day of each month the LHO shall report to the SBH:

    1. The original copy of each certificate of birth, of death, or still birth.
    2. A certification that no other births, deaths, or stillbirths occurred within his jurisdiction
    3. Any other information required by SBH

  • The LHO shall, before the 20th day of the month, furnish to the county auditor the records of all deaths that occurred during the prior month.

  • The LHO shall, on the last day of the month, furnish to the county auditor the records of all deaths that occurred within the officer's jurisdiction during the prior month.
