PA and PAUM

  • In case the alleged father of a child born illegitimate, marries the mother of the child, a new certificate filed within 6 months after such marriage may be accepted and filed in lieu of the first record which shall be removed from the files.

  • If the alleged father marries the mother of an illegitimate child, a new certificate filed within 6 months after the marriage on request of either parent or their agent for the purpose of using the father’s surname.

    1. Correct his local record according to the new certificate
    2. Remove all trace of illegitimacy

  • Early PA

    A man is presumed to be a child's biological father if he and the child's mother execute an affidavit on a form provided by the ISBH acknowledging paternity no later than 5 days after the child's birth and the person in attendance at birth files the affidavit with a local board of health

  • If a PA is filed with the LHO, the LHO shall correct the local record of birth by adding the name of the father to the certificate

  • LHD PA

    A PA may be executed in the LHD as well as the hospital.
  • PAUM

    If a man claiming to be the child's biological father marries the mother of a child born out of wedlock, they may produce proof of the marriage and execute a paternity affidavit.
  • A new certificate is no longer required for a PAUM

  • All evidence of the fact that the child was born out of wedlock must be removed from the child's record of birth.

  • Conclusively establishes paternity

    Except as provided in t in 16-37-2-2.1 (I), if a man has executed a paternity affidavit in accordance with this section, the executed PA conclusively establishes the man as the legal father of a child without any further proceedings by a court.
